Case Manager must complete the Hollywood Community Housing Corporation's pre-application on client's behalf. The original needs to be mailed to 5020 Santa Monica Blvd., Hollywood CA 90029. NO FAXES ACCEPTED. Once the pre-application has been received by Hollywood Community Housing Corporation, it will be reviewed to ensure eligibility and that all information is complete. If all criteria is met, the client will be placed on the Main Hollywood Community Housing Waiting List.
